Epidemiology: How Common Is Psoriasis?

Global Prevalence

Global Burden of Disease data tells a similar story from a different angle. Between 1990 and 2021, global psoriasis prevalence rose from about 23.06 million to 42.98 million cases, an 86% increase, while new cases climbed by 80%.

Disability linked to psoriasis has grown too. Over the same period, psoriasis-related disability-adjusted life years rose from 2.00 million to 3.69 million, an 85% increase, even as treatment options expanded.

Regional and Demographic Patterns

Psoriasis is not distributed evenly across the globe. The highest disease burden in 2021 was concentrated in high-income regions such as Western Europe and North America, where age-standardized prevalence rates exceeded 850 per 100,000 people.

By contrast, East Asia and sub-Saharan Africa showed substantially lower disease estimates, though researchers note this may partly reflect limited data collection rather than true lower rates. Newer modeling also shows disease onset patterns differ by sex.

Incidence rates among boys and girls are similar during childhood and adolescence, but incidence rises sharply with age among men later in life. Age-standardized incidence has been trending upward for decades, and this trend is expected to continue.

Etiopathogenesis: What Causes Psoriasis?

That trade-off is real and important. Active psoriasis involves body-wide inflammation, and the risk to the unborn baby from uncontrolled inflammation must be weighed against the risk from medication — a decision that should never be made without a dermatologist’s guidance.

Methotrexate and oral retinoids are strictly off-limits. Both are classified as contraindicated in pregnancy due to well-documented risks of birth defects, and effective contraception is essential while taking them and for a defined period afterward.

Identical twin studies offer some of the strongest evidence for how heavily genetics shapes psoriasis risk. When one identical twin has psoriasis, the other develops it in a large majority of cases, a concordance rate far higher than seen in fraternal twins raised in the same environment.

This gap between identical and fraternal twin outcomes is one of the clearest signals in all of dermatology that a disease has a powerful inherited component. Yet because concordance is high rather than universal, it also confirms that genetics alone does not fully determine outcome — environment and trigger exposure still matter.

Paradoxical Psoriasis

One of the strangest features of psoriasis is that the very drugs used to treat it can, in different patients, sometimes trigger it. TNF-alpha inhibitors, prescribed for conditions like rheumatoid arthritis or inflammatory bowel disease, occasionally cause a new psoriasis-like rash in patients who never had psoriasis before, a phenomenon known as paradoxical psoriasis.

Recognizing this pattern matters clinically, since these patients often go on to develop more classic skin or joint disease later. It is one more example of why a dermatologist’s trained eye, rather than assumption, is valuable even for seemingly minor or isolated symptoms.

Psoriasis as a Systemic Disease

Perhaps the single most important shift in psoriasis understanding over the past two decades is this: psoriasis is not confined to the skin. It behaves like a systemic inflammatory disease, similar in some ways to rheumatoid arthritis or inflammatory bowel disease.

Psoriatic Arthritis
