The Diagnostic Starting Point: Clinical Examination
For most patients, psoriasis diagnosis begins and ends with a thorough skin exam. Diagnosis of psoriasis is usually straightforward but may at times be difficult and challenging, depending on how classic or atypical the presentation looks.
What Doctors Look For
Dermatologists examine the shape, distribution, and texture of skin lesions closely. Typical psoriatic lesions are erythematous, scaly, and rounded plaques with sharply demarcated margins, most often located on the scalp, elbows, knees, and lower back in a symmetric, bilateral pattern.

Physician’s Global Assessment (PGA)
PGA offers a faster, more holistic snapshot of disease severity compared to the more detailed PASI calculation. PGA is a static 5-point morphological assessment of overall disease severity, ranging from 0 (clear) to 4 (severe), based on the clinical characteristics of erythema, plaque thickness, and scaling.
Because it’s quicker to perform, PGA sees frequent use in both clinical practice and research settings. Variations of the PGA are frequently used in clinical studies because it is a simple assessment that closely resembles what clinicians actually do during a routine visit.

Interestingly, the tool used most often for real-world treatment decisions isn’t always the most detailed one. The use of PASI by U.S. health care payers is very limited, especially for establishing treatment eligibility and access to biologics, where BSA is generally used instead.
Researchers have also explored combining measures for a fuller picture. A composite measure combining PGA and BSA has been shown to correlate strongly with PASI, suggesting it can serve as a simpler but similarly reliable alternative in appropriate settings.
Quality-of-Life and Symptom Tools
Severity scoring doesn’t capture everything that matters to a patient’s day-to-day experience. The Dermatology Life Quality Index (DLQI) captures how psoriasis affects daily functioning, sleep, relationships, and emotional wellbeing, complementing the physical severity scores above.

Validation studies support how well this simple tool performs in practice. In one validation study, a PEST score of 3 or higher showed 81% sensitivity and 79.7% specificity for detecting psoriatic arthritis, with an overall accuracy of 80% compared against formal rheumatologic evaluation.
What Increases the Chance of a Positive Screen?
Certain features tend to cluster with confirmed psoriatic arthritis and can raise clinical suspicion even before formal joint imaging. Patients with psoriatic arthritis had a significantly higher prevalence of dactylitis, nail psoriasis, and a PASI score of 10 or higher compared with those without joint disease.

When psoriatic arthritis is more formally suspected, rheumatologists use a separate, more detailed set of diagnostic criteria. The Classification Criteria for Psoriatic Arthritis (CASPAR) combines evidence of joint inflammation with supporting features like psoriasis history, nail changes, and specific imaging findings to confirm the diagnosis.
Lab Testing Before Starting Systemic Treatment
Once a diagnosis and severity level are established, patients being considered for systemic or biologic therapy need additional testing. This isn’t about diagnosing psoriasis itself, but about ensuring these more powerful treatments can be used safely.
Tuberculosis Screening
Of all the pre-treatment tests, tuberculosis screening has the strongest evidence behind it. TB screening with an interferon-gamma release assay is the laboratory test with the strongest evidence, graded B, supporting its routine use before starting systemic or biologic therapy.
Baseline screening for latent tuberculosis is recommended for all patients starting immunosuppressive therapy, and all patients starting a TNF-alpha inhibitor specifically should be screened via thorough history and testing before treatment begins. Testing methods have evolved over time toward more precise blood-based options.
Traditionally, latent TB screening was performed using a tuberculin skin test, but this is increasingly being replaced by the more sensitive and specific interferon-gamma release assay, also known as the QuantiFERON-TB Gold test. A chest X-ray is often added to this workup as well.

Not every commonly ordered test rests on equally strong evidence, and it’s worth understanding this nuance. Beyond tuberculosis screening, the evidence base for most other baseline screening and monitoring tests is comparatively weak, with many recommendations coming from clinical trial protocols rather than dedicated controlled studies.
This doesn’t mean these tests are unnecessary; rather, it means individual physician judgment, tailored to each patient’s specific risk factors, plays a larger role than a single universal checklist. Providers are encouraged to understand the evidence behind each specific medication and determine which tests are appropriate for each individual patient rather than following one blanket algorithm.
Diagnosing Psoriasis in Special Situations
A few patient groups and disease presentations call for extra diagnostic care. Recognizing these situations helps avoid delayed or incorrect diagnosis.
Children
Because pediatric psoriasis can resemble diaper rash, eczema, or seborrheic dermatitis, diagnosis in children sometimes takes longer to confirm. A pattern that fails to improve with standard treatment for these more common conditions, especially alongside a family history of psoriasis, should prompt referral to a pediatric dermatologist.
Skin of Color
