Guselkumab (Tremfya) Explained: India Pricing, Efficacy Data & Who It’s For

Guselkumab, marketed internationally as Tremfya, is part of a newer generation of psoriasis biologics that target interleukin-23 (IL-23) rather than IL-17. It’s known for a dosing schedule that’s easier to live with than many alternatives — but Indian patients researching it need to understand the current access reality before assuming it works like a locally stocked drug.

What Is Guselkumab and How Is It Different From IL-17 Inhibitors?

Guselkumab is a monoclonal antibody that blocks IL-23, a cytokine that sits “upstream” of IL-17 in the psoriasis inflammatory pathway — it essentially tells the immune system’s T-cells to stop producing the IL-17 that drives plaque formation in the first place, rather than blocking IL-17 directly. This mechanistic difference doesn’t necessarily make it more or less effective than IL-17 inhibitors like secukinumab, but IL-23 inhibitors as a class tend to show very durable, long-lasting responses and require less frequent dosing once maintenance begins.

Guselkumab’s Availability Status in India

Guselkumab (Tremfya) has not yet been registered or commercially launched in India by its manufacturer, Janssen (part of Johnson & Johnson). As with a few other newer-generation biologics, Indian patients currently access it primarily through a Named Patient Program — an import mechanism regulated by the CDSCO that allows a licensed doctor to prescribe an unapproved-in-India drug for one specific patient, with the necessary import permits arranged by a specialty pharmacy facilitator. This is a legitimate and legal pathway, but it is not the same as walking into a hospital pharmacy and having the drug on the shelf.

Guselkumab Dosing

The standard labeled dosing schedule is:

  • 100 mg by subcutaneous injection at Week 0
  • 100 mg again at Week 4
  • 100 mg every 8 weeks thereafter as maintenance

This works out to just six maintenance injections a year after the initial loading doses — one of the least frequent dosing schedules among approved psoriasis biologics, which some patients find meaningfully easier to sustain long-term than a monthly or twice-monthly injection.

How Well Does Guselkumab Work?

In its pivotal VOYAGE trials, guselkumab produced high rates of PASI 90 response (roughly 70%+ of patients) at week 16, and — notably — response rates continued to improve through week 48 in many patients rather than plateauing early, which is a pattern often seen with IL-23 inhibitors. Long-term extension data has also shown durable responses maintained over several years of continued treatment.

Cost Considerations for Indian Patients

Because guselkumab reaches India through import channels rather than local retail stock, per-unit cost is typically quoted “on request” by specialty facilitators rather than published as a fixed price, and will include import duties and facilitation fees on top of the manufacturer’s price. Who Is Guselkumab Typically Considered For?

Guselkumab is approved for adults with moderate-to-severe plaque psoriasis and active psoriatic arthritis. It may be discussed as an option for patients who want infrequent dosing, have not responded adequately to other systemic treatments, or have specific contraindications to IL-17 inhibitors (such as a personal history of inflammatory bowel disease, since IL-23 inhibitors as a class do not carry the same IBD caution).

Side Effects

The most common side effects reported in trials were upper respiratory infections, headache, injection site reactions, and joint pain. As with all biologics, screening for latent tuberculosis and active infections is standard before starting.
